Community-based Medicaid allows eligible seniors and individuals with disabilities to receive healthcare and long-term care services while continuing to live at home.

We help clients understand the reason, identify missing or incorrect information, gather supporting documents, and prepare a response or corrected application. When necessary, We can also help clients understand the next steps for requesting reconsideration or a fair hearing

We assist families navigate nursing home placement and the Medicaid application process, including reviewing eligibility, gathering financial records, preparing and submitting required documentation, and communicating with the facility and Medicaid office throughout the process. Currently there is a 60 month look back for NY Chronic Based Medicaid.  

We provide emergency Medicaid planning and application support for families facing an immediate need for home care or nursing home placement. We help assess eligibility, identify urgent planning steps, organize required documents, and guide the application process to prevent unnecessary delays. 

We assist eligible clients establish a Pooled Income Trust, complete the required paperwork, and coordinate with the trust organization. This planning option may allow individuals with income above Medicaid’s limit to maintain eligibility while using excess income for approved living expenses.

We offer Medicaid pre-planning services to help individuals and families prepare for future long-term care needs. This includes reviewing income and assets, explaining eligibility requirements, identifying appropriate planning strategies, and creating a personalized plan designed to protect resources and avoid preventable delays.
