Nursing Home Placement and Application Support
We assist families navigate nursing home placement and the Medicaid application process, including reviewing eligibility, gathering financial records, preparing and submitting required documentation, and communicating with the facility and Medicaid office throughout the process. Currently there is a 60 month look back for NY Chronic Based Medicaid.
